Obtaining Police Certificate
Unavailable for applicants inside and outside Indonesia. Indonesian police certificates require initial processing at a local or village level, which is nonstandard in nature, can be subject to capricious decisions by officials and may require nonofficial payments to proceed further.

Medical Exam Details
She should arrange to have her medical at latest 1-2 weeks before her consulate interview. She should print out the 4 medical forms, and fill in her biographical information on the form DS-2053. She should also bring to the interview her passport, one photo, her vaccination records and sufficient funds to pay the clinic’s fees.

At the end of the medical they should either tell her they are sending the results to the embassy for her, or provide her with a sealed envelope of results and X-rays. She MUST NOT open the envelope. She should take it to the interview, still sealed.

It is recommended she brings her vaccinations up to date while at her medical appointment. Having all the required vaccinations is required later when she applies for her Green card in the USA, so by doing it now she will save herself time later.

4. The enclosed Vaccination Documentation Worksheet (DS-3025)
5. Your passport and passports of all family members applying for visas
6. One photograph of each applicant

NOTE: Medical Examination Forms are only available at U.S. Embassy Jakarta.

We suggest you do to the medical examinations minimum of one week prior to the document submission. All visa applicants regardless of age need a medical examination. Children under age of 15 will not need a chest x-ray or serological test unless required by the examining physician. Result of the examination including x-ray will be delivered in one week by the examining physician to the Embassy.

Medical examinations are taken at your own expense. They remain valid for purpose of visa issuance for six months.

X-RAYS

1. For Pregnant Women. If you suspect that you are pregnant, please advise the examining physician in order that special precaution can be taken if a chest x-ray is require.

2. Previous X-rays. While previous x-rays are not normally required medical clearances, applicants who have them in their possession are advised to bring them to the clinic. This will save considerable inconvenience for applicants who need their previous x-ray results, as a decided by the examining physician for comparison purposes.

VACCINATIONS

If you have written record of vaccine history you may present your record to the examining physician to be determined whether you have received a complete series of each required vaccine.

** HIV test is no longer required by Jakarta Embassy and should not be given as part of the visa medical exam.

** As of April 2009, the base fee is still Rp 800,000. With vaccinations the total fee will come to about Rp 1,200,000, Although that may vary per person.
